Legacy Silverton Hospital is a 48-bed hospital located in Silverton, Oregon, that was founded in 1917. Legacy Silverton primarily serves the rural communities in northern Marion County including Silverton, Gervais, and Scotts Mills.
Legacy Silverton Hospital partnered with Marion County Public Health, Polk County Public Health, the former coordinated care organization Willamette Valley Community Health, and other community partners to gather data and implement an assessment of local health needs and local health infrastructure. The resulting data was used to inform their Community Health Needs Assessment and the Community Health Improvement Plan.
