Columbia Memorial Hospital is a 25-bed hospital located in Astoria, Oregon founded in 1880. Columbia Memorial Hospital serves the communities on the north coast of Oregon and south coast of Washington situated around the lower Columbia River estuary. Columbia Memorial Hospital is also a critical access hospital with a level IV trauma center.
Columbia Memorial Hospital works with community partners to conduct a Community Health Needs Assessment and Community Health Improvement Plan. Columbia Memorial Hospital also participated as a partner in the Columbia Pacific Coordinated Care Organization’s regional health assessment and regional health improvement plan.
