All fully completed CCPCs for each property are due at the end of February each year.
Annual Certification of Continuing Program Compliance (CCPC) are forms and other documentation that must be completed and submitted by affordable rental housing property owners and property managers to OHCS. Property owners must submit these forms annually to demonstrate compliance with OHCS administered programs for the previous calendar year. Below is information to help you through the process.
Where are the CCPC forms?
The CCPC Completion Guide and all forms are located in the
CCPC folder inside each property Procorem WorkCenter.
Procorem is the secure file-sharing system used by OHCS. Partners can access and download annual reporting CCPC forms and upload completed forms in Procorem.

If you are unsure if you have a Procorem account with access to your property's WorkCenter, or have had staff changes in the last year,
please email OHCS.CCPC@hcs.oregon.gov immediately.
CCPC Forms
This is a list of forms you will be able to access through your
Procorem WorkCenter. Each funding program has its own form. The forms you are required to fill out depending on the program your property is funded with.
