Annual Certification of Continuing Program Compliance (CCPC) are forms and other documentation that must be completed and submitted by affordable rental housing property owners and property managers to OHCS. Property owners must submit these forms annually to demonstrate compliance with OHCS administered programs for the previous calendar year. Below is information to help you through the process.
Overview
Owners of affordable housing who have received funding through OHCS are required to certify annually that each of their properties have been in compliance with each program’s regulations during the entire compliance (Certification) period; for the entire compliance preceding calendar year.
The owner’s CCPC is the reporting format required by OHCS. This reporting is required annually for all programs and is due on or before February 28 (29 in a leap year).
A certificate for each program (funding type) provided to the owner for each property must be completed and submitted to OHCS by the due date. For example: If the property has multiple funding types, then a certification for each of these programs must be completed in their entirety (including all required additional documentation) for the property and submitted to OHCS.

The CCPC Completion Guide, forms, monitoring reports, and other supporting documents are located in the
CCPC folder inside each property Procorem WorkCenter.
Procorem is the secure file-sharing system used by OHCS. Each property has a Procorem WorkCenter where partners can access, download, and upload annual reporting CCPC forms and documents.
