Disciplinary Actions Taken by the Board

The Oregon Board of Pharmacy meets six times a year to review compliance cases and take disciplinary action as needed. This report summarizes the disciplinary actions taken during the specified time period. To review a list of these actions, select the records in the table below. 

For transparency, the Oregon Board of Pharmacy Online License Lookup portal allows individuals the ability to check the license status and any disciplinary documents for licensees and registrants. Please note that many licensees and registrants have similar names, please review the information closely to ensure it is the correct record.
