Pharmacy Closure Notification Forms & Information

Visit our Pharmacy Closures page, select and complete the form for the appropriate type of closure: 

  • Temporary or Emergency Pharmacy Closure
  • Permanent Pharmacy Closure 
  • Non-Resident Permanent Pharmacy Closure 


Drug Compounding Notification Form

Drug Outlets that initiated compounding on or after 6/17/2026 are required to comply with OAR 855-183-0010. Simply complete and submit the form below. 


Drug Outlets that initiated compounding prior to the effective date of the rules do not need to complete and submit the form above. Entities who were engaged in compounding before 6/17/2026 will need to ensure that the outlet's compounding status is listed properly within their eGov profile. 

To verify or update compounding status: 

Visit mylicense/eGov

  1. Log in using the User ID and password for the drug outlet registration record
  2. Select “Update License Info & Upload Documents” on the left-hand side of the screen
    • Next to the license/registration you are updating, select “Continue”
  3. Follow the prompts to move through each page of the update process. Review and update information as needed. On the “Information Update” page, go to the “Compounding” section and select or de-select all that apply.
  4. Continue following the prompts through the pages. On the “Finish” page, review the information summary.
    • If any information is incorrect, go back to the relevant page in the menu and update
    • Once everything is correct, select "Submit Attestation"
    • Return to the Home page and select "View Documents" to view or download the submitted information

HPSP (Health Professionals Services Program)

The purpose of HPSP is to provide a statewide confidential monitoring program for licensed healthcare professionals who have been diagnosed with a substance use disorder and/or mental health disorder. 

HPSP has two important goals: 
  • Supporting public safety 
  • Assisting health professionals (licensees) with substance use disorder and/or mental health issues so that they may continue working in their chosen profession
