Coordinated Care Organizations (CCOs)

About CCOs and the Oregon Health Plan (OHP)

OHP's local health plans are coordinated care organizations or CCOs. CCOs work with doctors, counselors, dentists and other local providers. You choose your CCO when you apply for OHP. 

  • Except for extra services and pharmacies, all CCOs offer the same OHP benefits.
  • Extra services can be items or services to support your health or manage a condition. Some extra services are available to all members. Others are available on a case-by-case basis or by special request. You can contact each CCO for a full list of services and how to access them.

​Instead of just treating you when you get sick, CCOs work with you to keep you healthy. CCOs can help you manage your health conditions. There may be added services for members with chronic conditions such as diabetes, asthma and heart disease, or for those with other health needs.

  • ​CCOs may give and cover some benefits that OHP does not cover, such as weight loss classes.
  • You and your family can get medical, dental and behavioral health care when you need it.​
CCOs can:
  • Give you a health care team to work with
  • Help your caregivers and family members join your team
  • Help you avoid testing or services you already had or don’t really need
  • Give you the tools and support you need to stay healthy and
  • Give you advice that is easy to understand and follow.​

​American Indians, Alaska Natives, and members with Medicare have other options.

  • American Indian and Alaska Native members can join, change or leave their CCO at any time. (You will still need to choose a CCO for dental care.)
  • Members with Medicare can change or leave the CCO they use for medical care at any time. Medicare members must use a CCO for dental and mental health care.
  • If you don't want to be in a CCO, you can call 800-273-0557 and tell us.

​Call the doctor’s office or clinic you have now (if you have one). Ask which OHP plan the provider takes.

Find out if the CCO’s providers are near where you live.

Find the CCOs listed for your area. Call them or go to their websites to learn which doctors, nurses, clinics, hospitals, pharmacies, dentists and mental health providers you can use.​

​OHA will choose a CCO for you unless you are American Indian or Alaska Native. OHA will choose a CCO in your area that is open to new members.​

If you are enrolle​d in Medicare

OHA will enroll you in a CCO that covers medical, dental and behavioral health care. You don’t have to get your medical care through the CCO. If you want to change this, you need to tell us.

If you are American Indi​an or Alaska Native

OHA will not enroll you in a CCO. OHA will pay for your medical, dental and behavioral health care. You don’t have to get your medical care through the CCO. If you want to change this, you need to tell us.​ You can fill out the OHP 7​​​20 form to do this.​

How to Compare Plans

Learn how to use the list below to compare CCOs and choose a CCO:

In the search field, type in the name of your county. Do not add punctuation, extra space(s) or the word "county" when typing the name.

CCOs that serve your county will have your county listed in the "Service Area" field.

"All ZIP codes" means the CCO serves the whole county.

If certain ZIP codes are listed for your county, please read carefully! This means the CCO:

  • Serves only those ZIP codes; or 
  • Serves all parts of the county except for​ those ZIP codes.

​To find CCO providers, view the CCO’s Provider Directory. You can search for specific providers by name, specialty or location.
