Submitting Specimens to CDC for Testing

Requirements for All Specimens Submitted to CDC

All specimens submitted for testing at the Centers for Disease Control and Prevention (CDC) must be accompanied by the appropriate forms, detailed below. Without these forms, specimens cannot be forwarded to the CDC for testing.

How to submit a specimen for testing at the CDC
  1. Complete the CDC's Specimen Submission Form 50.34 electronically (preferred)
      • If you cannot type into this form, your browser may not support the fillable form. Try saving the form to your computer to type into the form.
      • If you must use a hand-fillable form (not preferred) you may print the blank form then complete by hand. Include the completed form with the specimen(s) and submit to the OSPHL.
  2. Print the completed form.
  3. Complete the appropriate OSPHL Test Request form sent with your specimen collection kit or available on the OSPHL Forms and Collection Kits page.
  4. Enclose both completed forms with your specimen and submit to the OSPHL.
