Funeral Home Orders

Funeral home staff order death certificates from the state vital records office by completing the funeral home order form and submitting it along with a copy of their valid identification. Documents should be emailed to CHS.FuneralHomeOrders@oha.oregon.gov or faxed to 971-673-1203.

Certificates can only be ordered for records where your facility is the funeral home on the death record. Orders are typically completed within 1-2 weeks and fees for the certificates are billed to your funeral home monthly. If certificates need to be exchanged after an amendment, the funeral home order form, valid identification and certificates to be exchanged should be mailed to:

Helpful tips:

  • Make sure funeral home staff ID is unexpired, unredacted, and (if faxing) enlarged and lightened.
  • Be sure the decedent information is complete and correct so the proper record can be located.
  • Remember to sign and date the order form.
  • If you have questions about the status of your order, please email: CHS.FuneralHomeOrders@oha.oregon.gov – do not resubmit the order form to the state or you may be billed for duplicate orders.

County and Hospital Outreach for Funeral Homes

Funeral homes may now fax Form 45-106, the 24-Hour Notice of Receipt of Body, directly to the county of death. Provided here is a list of fax numbers for county vital records offices (PDF, updated April 2014).

When sending a hybrid or fully paper report of death to a medical certifier for signature, you may include this Electronic Registration of Death Records brochure (PDF, updated 06/2022) to promote the use of our electronic records system.

If a death has occurred in a medical facility, a decedent contact may be available to answer questions about referral of death certificates to a medical certifier in OVERS. Please consult the following list of medical facility decedent contacts (PDF, updated July 2014).


Indigent Disposition Fund and Reimbursement for Services

The purpose of the Indigent Disposition Program is to provide licensed funeral homes with reimbursement for services for the disposition of unclaimed indigent bodies. This is a limited fund created to offset the costs incurred in providing final disposition.

The Oregon Legislature passed a bill to move administration of the fund and reimbursement program to the Oregon Mortuary and Cemetery Board beginning in 2016.
