Oregon Psilocybin Services - Information for Licensed Facilitators

2026 Requirements

On October 24, 2025, OPS adopted new and updated administrative rules (please see: OPS Rules with Tracked Changes).  On November 4, 2025, OPS sent a Letter to Licensed Facilitators describing new and updated rules effective January 1, 2026. OPS also sent a letter describing rules related to Product Labels and Potency.

As part of new 2026 requirements, OPS developed an updated Continuing Facilitator Education Hours Log for use beginning January 1, 2026 (more information about Continuing Facilitator Education Requirements below). OPS also updated several operational forms that must be used beginning January 1, 2026:

OPS updated the Client Information Form to include information related to facilitators who hold other professional licenses issued by boards identified in recent legislation, HB 2387.


OPS updated the 303 Client Data Form to comply with HB 2387 which amends ORS 475A to require the collection of 'veteran status' of clients. The 303 Client Data Form was updated to include new categories in the racial and ethnic identity questions to reflect the REALD and SOGI Demographic Data Collection Standards developed by the OHA Equity and Inclusion Division. 


OPS updated the Psilocybin Product and End of Session form to include information related to facilitators who hold other professional licenses issued by boards identified in recent legislation, HB 2387.


Based on rules adopted in 2025, OPS updated the Client Bill of Rights to remove the right "To receive prior notice of any service center licensee representatives who may be present in the client administration area to assist with operations".
 

Renew a Facilitator License

All psilocybin license types have a one-year term, beginning on the date the license was first issued.

To ensure license continuity, you must submit a complete renewal application at least 60 days before your license expires. See OAR 333-333-4250.

  • Renewal applications that are complete, submitted according to requirements, and submitted at least 60 days before the license expires will be considered timely under OPS rules. Licensees who submit timely applications may continue to operate on an expired license if OPS is unable to process the renewal application prior to the license expiration date. Incomplete renewal applications will not be considered timely unless they are made complete at least 60 days before the license expires. 
  • Renewal applications that are submitted less than 60 days before the license expires will be considered untimely under OPS rules. Licensees who submit untimely applications are prohibited from continuing to operate on an expired license if OPS is unable to process the renewal application prior to the license expiration date. Licensees are prohibited from exercising license privileges on the date their license expires if OPS is unable to process their renewal application before the expiration date and the renewal application is untimely.
We encourage licensees to begin the renewal process as soon as the renewal window in TLC opens. The renewal window in TLC opens 120 days prior to your license expiration date.

Continuing Facilitator Education Requirements

Beginning January 1, 2026, facilitator licensees must report at least four hours of continuing education during their annual license period to be eligible for license renewal. As part of renewal application requirements, licensed facilitators must provide documentation of the education received.

OPS will consider continuing education (CE) hours to be valid if:
(1) the CEs are completed within the one-year period prior to renewal date, 
(2) they meet the requirement in Oregon Administrative Rule 333-333-3300, and (3)  the education was offered by:
  • A training program with curricula approved by the Authority, or
  • An organization or conference that has been approved to offer continuing education for other types of professional licenses or certification.
  • Beginning January 1, 2026, facilitators who provide qualifying continuing education may report those hours to meet their own reporting requirements

Licensed facilitators will need to include this signed form as part of the renewal application process.

Informational Fact Sheets

Beginning January 1, 2026, a facilitator licensed under ORS 475A.325 who is also licensed by one of the seven boards named in House Bill 2387 may conduct preparation and integration sessions under both licenses. Change Requests

Licensees are required to use the forms below for change requests. These forms must be submitted to the OPS Licensing team through the Training program, Licensing and Compliance (TLC) system. Change of Name
To request a change of a legal name, facilitators must submit a completed Change of Name Form.


For any change requests not covered by these forms, please message the licensing team in TLC messenger with your request.

License Surrender Requests

Psilocybin Product Potency Information

This Psilocybin Product Potency Information Document is designed to support client decisions on selecting a product type and dose that aligns with their intentions. The document explains different terms used in psilocybin product testing and where they can be found on product labels and other documents required to be shared with clients. OPS encourages facilitators to share and discuss this information with clients during preparation sessions.

Operational Forms for Facilitator Licensees

Oregon Psilocybin Services (OPS) administrative rules require that licensees use certain forms and documents for the production of psilocybin products and the provision of psilocybin services. Please consult the rules to determine whether a form must be competed for every client or whether it must be completed only when certain conditions or events occur.
