Consolidated Plan: Guidance to Public Housing Authorities and Continuums of Care

The following certification guidance is for public housing authorities and continuums of care to ensure their U.S. Department of Housing and Urban Development (HUD) plans are consistent with the State Consolidated Plan.

Public housing authorities (PHA’s) are required to submit a signed certification that their 5-year and/or Annual PHA Plan is consistent with the Consolidated Plan covering their jurisdiction. Continuums of Care (COC) are also required to submit Certification of Consistency with the Consolidated Plan when they complete their COC Notice of Funding Availability (NOFA) application. This document is designed to provide guidance to agencies covered by the State of Oregon “Balance of State” Consolidated Plan.

Public Housing Authorities Guidance

For public housing authorities, OHCS requires the following documents be submitted:

  • Copy of your completed 5-Year PHA Plan (if applicable).
  • Copy of your completed Annual Plan
  • A copy of any policy changes made during the year as outlined in your 5-year or PHA Plan
  • Any supporting information relevant to your 5-Year and/or Annual PHA Plan submission
  • HUD-50077-SL
  • Completed "PHA Consolidated Plan Consistency Questionnaire"

Completed information should be emailed to glenn.muna@hcs.oregon.gov.

Please allow 10 business days for review and signature authorization. Incomplete submission will be returned and will not be processed until a complete request has been submitted.

Continuums of Care Guidance

Continuums of Care (CoC) should submit the following information for review:

  • Copy of your COC NOFA Application
  • Completed form HUD-2991.
    • Please note: Do not fill out Certifying Official Name or Title.
  • Completed "COC Consolidation Plan Consistency & Collaboration form"
  • Any supporting information relevant to your COC NOFA Application

OHCS will be verifying that all submittal requests are consistent with the most recent OHCS Consolidated Plan and the applicable Analysis of Impediments. Submitted request should be consistent with these documents found on the state’s Consolidated Plan webpage:

  • Oregon’s 2021-2025 Consolidated Plan
  • State of Oregon Analysis of Impediments
