If you have been licensed as a pharmacist in another state and meet all of the requirements listed in ORS 689.265 and OAR 855-019-0130, you will apply for licensure via Reciprocity (Licensure Transfer).
If you are applying for licensure via Reciprocity, visit mylicense/eGov to "register"/create a new personal account to apply for a pharmacist license. Be sure to select "Reciprocity" as the obtained by method.

If you have previously held any type of license with the Oregon Board of Pharmacy and cannot find your record when you search for an "Existing Personal/Professional Record", please contact the Board for assistance @ pharmacy.licensing@bop.oregon.gov
*If you are a Foreign Pharmacy Graduate or will be attending an Oregon PGY1 or PGY2 program, and have not been licensed as a pharmacist for one full year, email the Board for more information on how to apply for a license via Reciprocity.
