Fidelity Wraparound is a voluntary care coordination model and planning process available to children, youth and young adults on Oregon Health Plan (OHP). It offers a planning process to develop a unique set of community services and supports individualized for a youth and family to achieve positive outcomes.

How Fidelity Wraparound Works
Youth, young adults and their families will work with a team trained to support their needs. Together they will create a plan of care, based on what the youth and family think will be most helpful. This plan helps youth and family members move toward their goals and vision for the future.
Who Is Fidelity Wraparound For?
Wraparound is for children, youth and young adults under age 21 who:
- Experience complex mental health issues and
- Are involved in two or more systems (e.g., behavioral health, foster care, special education, juvenile justice, intellectual/developmental disability programs), and
- Are currently receiving OHP.
