New Sex Designation for Marriage Form

  • The Center for Health Statistics (Vital Records) is excited to announce that the Application, License and Record of Marriage has new options for sex designation.  Each Party will have the option to choose Female, Male or X.  X, indicating nonbinary, has been added to include individuals who are not exclusively male or female. This does not require the form to be changed.

Electronic registration system

  • The Center for Health Statistics is launching an electronic system for registering and filing marriage records at the county level, in partnership with Helion and VitalChek. Please see our County Marriage Feed page for more information.

Changes to marriage license form

  • With the passage of House Bill 2113 and Senate Bill 751A the Application, License and Record of Marriage form (45-4) is changing. The newest version of the form will go into effect January 1, 2018.

New instructions for officiants and couples completing the marriage form

  • The Center for Health Statistics has updated the instructional documents for both officiants and couples to use when completing the marriage form. See below for more information.

The following documents provide guidance to couples, counties, and officiants when filing an Oregon certificate of marriage. Please note that a new Application, License and Record of Marriage form (45-4) goes into effect January 1, 2018.
