DUII Services

To Get a DUII Treatment Completion Certificate:

DMV Form 735-6821 (DUII Treatment Completion Certificate) is required to reinstate Oregon driving privileges for anyone under a diversion agreement for or convicted of driving under the influence of intoxicants (DUII). To get a DUII Treatment Completion Certificate, you must:

  • A screening interview with an Oregon Alcohol and Other Drug Screening Specialist (ADSS), and 
  • A DUII services program. The ADSS will provide information about how to find an approved provider and refer Oregon residents to the provider they choose. Oregon DUII Services providers will issue the certificate upon completion of the program. If you are from out of state and complete a program outside Oregon, you must ask Oregon Health Authority (OHA) for the certificate.

 Please note: Enrolling in a program that is not certified to provide DUII services may not satisfy court or DMV requirements. Oregon residents should check the Substance Use Disorders Services Directory to ensure the program is provided by an approved DUII Services Provider.

How to Find DUII Services

​Each court in Oregon has at least one Alcohol & Other Drug Screening Specialist (ADSS). The ADSS screens people who have received a DUII. View​ the current ​ADSS Directory.

After the screening, the ADSS will:

  • Provide a list of local certified DUII Services Providers for the individual to choose from. Read about Your Right to Choose a DUII Services Provider; and
  • Send referral paperwork to the DUII Services Provider chosen by the individual.

The provider will assess people for substance use and the type of DUII services they need: 

DUII Education is for people with previous DUIIs, who do not have a substance use disorder. If referred to DUII Education, you must:

  • Complete at least 12 hours of education over a minimum of 4 sessions over 4 weeks, and 
  • Abstain from use of intoxicants, except as allowed in Oregon Revised Statute (ORS) 813.200.

DUII Rehabilitation is for people with a previous DUII or who have a substance use disorder. If referred to DUII Rehabilitation, you must:

  • Complete the 12 hours of DUII Education 
  • Complete any other substance use disorder treatment services deemed necessary, and 
  • Abstain from use of intoxicants, except as allowed in ORS 813.200 for at least 90 days. The 90 days of abstinence must occur outside a controlled environment.

People who live outside Oregon may be referred to a DUII services program that is licensed or certified by their state. 

People who live in a state that does not license or certify DUII service providers must enter and complete a program that is substantially equivalent to what Oregon requires.
