About the OYSO Survey
What is the Oregon Youth Survey Online (OYSO)?
OSYO is an online survey administered each year to a randomly selected group of Oregon teens, aged 15-18. The survey asks questions about behaviors, attitudes and perceptions related to alcohol, tobacco, marijuana and other health topics. The information we gather helps us better support youth and communities across Oregon.

Launches in June 2026: If you received a letter in the mail, we’re inviting your teen to take part in this year's survey.

Will reach at least 4,500 Oregon teens. A combination of mailed invitations and an online panel will be used. This helps us make sure our data accurately represents Oregon.
Your teen may have been selected because:
- Your household was randomly chosen to receive an invitation by mail.
- You are part of a web panel survey, where you occasionally agree to take online surveys.
Why this survey matters
HPCDP regularly invests in new and better ways to collect data, with a goal of reaching groups that are traditionally hard to reach through standard population health surveys. Youth are one such important group.
Teen voices help shape programs, guide health policies, and set priorities for Oregon. The more youth who participate, the better we understand what Oregon teens need to thrive. By sharing their voice, your teen plays a key role in creating a healthier future for Oregon communities.
What to expect
Taking part in the survey is easy.
- The survey takes about 15 minutes to complete.
- You can take it on your phone, tablet, or computer — whatever works best for you!
- You don’t need to prepare anything. Your teen will just need the code included in their invitation letter to get started.
- If invited, your teen can choose to receive a gift card for participating.
Honest feedback from your teen plays a major role in shaping decisions that affect our community!
Your privacy matters
We take your family's privacy seriously:
- Your teens'
responses are confidential and will never be linked to their name, address, or any other identifying information.
- We will
never sell your data or share it beyond this project.
- Participation is
completely voluntary. Your teen can stop at any time.
- Taking or not taking the survey
will not affect any services your family receives.
