CEU Reporting Process

Effective January 1, 2016, DWS implemented a new CEU reporting process to improve renewal efficiency. CEU documentation is no longer submitted as part of the renewal process; instead, operators submit CEU Self Certification form, and OHA performs a CEU audit that is separate from the renewal process.  

Operators who are selected for a CEU audit will receive notice by postcard after the renewal period ends, generally in March or April each year. Selected operators will then have 30 days to submit copies of their training documentation for review. Acceptable documentation includes copies of training certificates, and copies of agendas and receipts for registration and payment at short schools and similar training events will also be considered. These documents may be submitted by mail or fax, and operators will be contacted by DWS if there are questions regarding their data.

Instructions

  1. When notified by mail that you have been selected for audit, send in copies (not originals) of the certificates for the specific training identified on your CEU Self Certification form during renewal. 

  2. Mail copies of training certificates to: Operator Certification, 800 NE Oregon Street, Suite 640; Portland, OR 97232. You may also fax the material to (971) 673-0694, ATTN Operator Certification. 

  3. Operator Certification staff will validate your training documents and will contact you if there are any questions.

  4. To receive written audit results please provide an email address when you submit your audit documents.
