When you pay online, make sure you email, fax or mail in your renewal paperwork (see below).

Individual renewal forms are mailed to all currently certified Distribution and Treatment operators in October of every year. If you didn’t receive your notice, download a renewal form below, or contact the program: dws.opcert@odhsoha.oregon.gov or 971-673-0321

It is your responsibility to provide a current mailing address and other contact information and to report any changes on your certification, such as change of mailing address, phone number(s), email, or employer. Update your contact information by emailing the program dws.opcert@odhsoha.oregon.gov or 971-673-0321

Renewal Requirements

CEUs: Operators are required to obtain 2.0 CEUs each certification cycle. Record your CEUs on the CEU Self-Certification Affidavit form, sign it, and turn it in with your signed renewal notice and payment.

Note: If you are a new certified operator this year, you do not need to complete the CEU Self-Certification Affidavit.

Pay by Credit Card: You can now pay your certification fee online with a credit card! You will receive a receipt from noreply@nicusa.com when you pay online. It will go to the email address you provide while paying.
