The PCH program recognizes the valuable work our LPHA partners do! We want to increase our collaboration between state, community-based organizations, and LPHAs

In 2026, we adopted a new model to provide a specific point-of-contact (Equity Strategist) for each region of the state. These contacts coordinate with the OHA Public Health Systems Consultants.

Here are some areas the Equity Strategists can collaborate on or support with.

Collaborating with CBOs/LPHAs

This dashboard can be used to view approved CBO work plans for the Public Health Equity Grant and Local Public Health Authority (LPHA) Program Element 51 Public Health Modernization work plans. These work plans are posted to support coordination and collaboration between CBOs and LPHAs serving the same geographic area. You will be asked to log into Smartsheet and request access. Someone on our team will grant access and you will be notfiied.
