Chronic Conditions & Risk Factors Data

Includes prevalence of chronic conditions, health protective and risk factors from the adult Behavioral Risk Factors Surveillance System (BRFSS) and surveys of youth.


The Below are resources provided by the Health Promotion and Chronic Disease Section (HPCDP) of the Public Health Division.

Priority Populations

Includes adult chronic conditions, risk and protective factors, and health screenings by race, ethnicity, sexual orientation and rural residency.

Includes adult cholesterol screening, colorectal cancer screening, diabetes screening, mammogram and others.

Student Wellness Survey

Includes state data on ease of availability, friend or parental disapproval, use and perceived risks from substances for 6th, 8th and 11th graders.

Includes adult arthritis, asthma, cancer, chronic obstructive pulmonary disease, coronary heart disease, diabetes, heart attack and stroke.

Includes state and county reports for 6th, 8th and 11th graders. Note that the Student Wellness Survey ended in 2018. There is no new data.
