Oregon Health Authority (OHA) Health Facility Licensing and Certification licenses and regulates the following health care facility types and requires FPS to review any new requests for licensure, certain modifications to existing licensure, and changes of use.

Hospitals and Hospital Licensed Facilities

Ambulatory Surgery Centers (ASC)

Extended Stay Centers (ESC) affiliated with an ASC

Freestanding Birthing Centers

Dialysis Facilities

Special Inpatient Care Facilities (SICF):

Substance Use Disorder Treatment Facility

Religious Nonmedical Health Care Institutions

Oregon Department of Human Services (ODHS) Safety, Oversight and Quality (SOQ) licenses and regulates the following facility types and requires FPS review of any new requests for licensure, certain modifications to existing licensure, and changes of use.
