Note: Please be advised that due to COVID-19, many DEQ staff are working remotely, at reduced hours, or both. We appreciate your patience in advance and will keep you apprised of the status of your request.

How to request records from DEQ

If you can’t find what you are looking for on our website, use:

Public Records Request Form     

  • Fill out the form as completely as possible, which helps us provide you with the information you need.
  • While we will accept requests with multiple sites, we recommend only one site per request. Limiting your request to one site will allow us to get you the records quicker and at a lower cost. When entering the site(s) address, please use the complete address (including city). We cannot search for something like, "1600 block of SW Smith Street".
  • We will contact you if we anticipate a fee for your request. DEQ requires payment before making records available. DEQ's Frequently Asked Questions provides more information on fees.
