Requirements for Recertification
To renew Full Certification, evaluators must submit all required materials, pay the recertification fee, and attend the one‑day recertification training
before their current certification expires.
Step 1: Email Required Materials
Send the following documents to Forensic.Certification@odhsoha.oregon.gov:
- Completed application (form below)
- Updated CV or résumé
- Proof of current Oregon license to practice
- Two redacted competency and/or criminal responsibility reports completed in Oregon within the past 24 months
- If requesting eligibility to perform criminal responsibility evaluations, at least one sample must be a criminal responsibility evaluation.
- If performing juvenile forensic evaluations, at least one sample must be a juvenile competency or criminal responsibility evaluation.
Step 2: Submit the Application Fee
The $250 non‑refundable fee must be paid by check payable to "Oregon State Hospital." Mail to:

Step 3: Register for Recertification Training
All evaluators must attend the one‑day
Oregon Forensic Evaluator Recertification Training. Go to
