Ordering By Mail

How to order

Send an order form with complete record information.

  • Cost:
    $25.00 nonrefundable fee for the first record. $25.00 for each additional copy of the same record ordered at the same time. Add $5 for a full image birth record from 1903-2007.
    Where date of event is unknown for death, marriage, or divorce records, $25.00 covers a five year search. Add $1.00 per year if more than 5 years need to be searched. Exact date of birth is required for birth record orders unless the record is more than 100 years old.

  • Processing Time:
    • 12-14 weeks for orders received by regular mail. 
    • Three to five business days for orders received by priority, certified, or overnight delivery, assuming no record problems are identified.

      Processing of the order will be delayed by at least three weeks if payment is by a check from a new bank account with a low check number, is a temporary check, is from an unrelated third party, or there are alterations to the printed information on the check. Rush Mail Orders

To speed processing of a mail order, send your request by any overnight delivery service to Oregon Vital Records, 800 NE Oregon Street, Suite 205, Portland OR 97232. If you need the record returned by overnight delivery, enclose a pre-addressed, prepaid overnight delivery envelope. Pay all fees with a money order rather than by check. Processing of orders may be delayed by three weeks if payment is by check. Orders may also be delayed for missing or wrong record information on the application, if the applicant ordering is not eligible, if the ID or signature of the applicant is missing, or if records are not yet registered. Corrections or amendments will add additional time for processing.
