Home Birth Registration

      How to Register a Birth Record for a Birth that Occurs outside of a Licensed Health Care Facility


    This includes any birth that occurs without a licensed attendant (doctor, physician associate, nurse practitioner, licensed midwife) or outside of a health care facility that is licensed by the state. OAR 333.011.0220 
    Summary information about the process and timelines of requesting and submitting a home birth packet are available on this handout.

    For Children Under the Age of One

    If you are planning to have a home birth in the near future, you must contact the Center for Health Statistics and complete the following steps for your child to get a birth certificate.  
    -or- 
    If you had an unassisted birth at home without a midwife or other licensed medical professional and your child is currently under the age of one (1), you must contact the Center for Health Statistics and complete the following steps for your child to get a birth certificate.

    1. Once you receive your home birth packet in the mail, read it carefully. It contains forms and information about required documentation needed to complete the registration of your child’s birth with the state and how to obtain a birth certificate. For your convenience, we have included a list of what you will receive in the home birth packet:


    If you do not register your child's birth before they turn one year old, the registration requirements change and may involve one or more of the following:

      • Additional Forms, documentation, and process.

      • Registration and other possible fees.

      • Your child not receiving a Social Security number

      • Longer wait times for documents to be reviewed and the birth certificate to be processed.

    Include all required documents and fees in a sealed envelope.

    For Anyone Age One (1) Year and Older


    If it has been more than a year since the home birth occurred, a different process called Delayed Registration of Birth must be completed.

Home Birth Registrations are processed by the Amendments Unit at Oregon Vital Records.

When you call or email an amendments specialist about registering your home birth, please leave one complete message and include the following:


  1. Your first and last name (Please spell if leaving a message.)

  2. Telephone number

  3. Email address​

  4. The date of the home birth 

  5. An order number (if applicable)
