Initial Applicant
As of January 1, 2018, all initial applicants must submit fingerprints for a FBI background check.
License Renewal Applicant
If you have lived, worked, or attended school for 60 or more consecutive days outside Oregon in the last five years, OHA requires a fingerprint based criminal history check. This includes license renewal applicants who have lived (or currently live), worked or attended school outside Oregon for 60 or more consecutive days since their last license was issued. *
Instructions
Do not make an appointment with Fieldprint prior to completing an EMS Provider License application. Fingerprint results received without a corresponding application may be declined and require resubmission.
- Print and follow the instructions provided in the Fieldprint Electronic form
- You must take this form to your appointment.
- Do not make an appointment with Fieldprint prior to receiving NREMT certification at the level in which you are applying. This applies to initial EMT, AEMT and Paramedic license applications.
- Fingerprint results may take up to 30 days to process.
*Note: If you work or attend school outside the state of Oregon and return home each night to Oregon, this is considered commuting and you do not need to submit fingerprints.
Late Fee: Renewal applications submitted after June 1 will include a $40.00 late fee in addition to the license renewal fee.
