Initial Applicants and License Renewal Applicants
As of January 1, 2018, all initial applicants must submit a fingerprint based criminal records check.
License Renewal Applicant
If you have lived, worked, or attended school for 60 or more consecutive days outside Oregon in the last five years, OHA requires a fingerprint based criminal history check. This includes license renewal applicants who have lived (or currently live), worked or attended school outside Oregon for 60 or more consecutive days since their last license was issued. *
- Print and follow the instructions provided in the Fieldprint Electronic form (pdf).
- You must take this form to your appointment.
- Do not make an appointment with Fieldprint prior to completing an EMS Provider License application. Fingerprint received without a corresponding application may be declines and require resubmission.
- Do not make an appointment with Fieldprint prior to gaining NREMT certification at the level in which you are applying (This applies to initial EMT, AEMT and Paramedic license applications.)
*Note: If you work or attend school outside the state of Oregon and return home each night to Oregon, this is considered commuting and you do not have to submit fingerprints.
Renewal: If you complete your online application by June 1, but do not submit your fingerprints and processing fee on time, your application will be considered incomplete and a $40.00 late fee will be assessed.
