Community health improvement plan review, implementation and evaluation

CCOs’ community advisory councils (CACs) partner with their local public health authority, local mental health authority, community-based organizations, academic institutions, and hospital systems to develop a shared community health assessment (CHA) process, which includes conducting the assessment and developing a community health improvement plan (CHP). CCOs must engage diverse representatives of critical populations and community stakeholders in the CHA, and they must identify health disparities in their communities. The plan adopted by each CAC must describe the scope of the activities, services and responsibilities the CCO will consider upon implementation. The first CCO CHAs and CHPs were completed in 2014, and CACs are required to publish annual progress reports for their improvement plans. For more details, see ORS 410-141-3145.
