Mission: The Oregon Health Authority (OHA) Transformation Center is the hub of innovation and quality improvement for Oregon’s health system transformation efforts to achieve better health, better care and lower costs for all.
Goals: The Transformation Center identifies, strategically supports and shares innovation at the system, community and practice levels. Through collaboration, we promote initiatives to advance the coordinated care model.
The center continues to adopt new strategies to further advance primary care, value-based payment, population health, behavioral health integration, oral health integration, incentive measure work and other cross-cutting supports.

Value-based Payment
Primary Care Payment Reform Collaborative
As initiated through Senate Bill 231, the Transformation Center coordinates a
Primary Care Payment Reform Collaborative. This collaborative convenes payers to share best practices on primary care value-based payment methods and aligning initiatives.
Comprehensive Primary Care Plus (CPC+)
Following Oregon’s selection as a CPC+ region, the center began supporting the
Medicaid fee-for-service CPC+ implementation. CPC+ is a regionally based, multi-payer advanced medical home model offering an innovative payment structure to improve health care quality and delivery. It is a five-year federal program that began in January 2017 and will bring significant Medicare dollars to Oregon.

Health-Related Services
Health-Related Services (HRS) are non-covered services that are offered as a supplement to covered benefits under Oregon's Medicaid State Plan to improve care delivery and overall member and community health and well-being. The Transformation Center
develops resources and offers technical assistance to guide CCOs through the HRS process.
Social Determinants of Health & Equity
Community advisory council (CAC) supports
The Transformation Center provides targeted supports and learning opportunities for
CCO community advisory councils (CACs).
Community health improvement plan (CHP) progress report analysis
The Transformation Center
collects and analyzes CCOs' CHPs and develops resources for CCOs and their collaborative CHA/CHP partners.
Supporting Health for All through REinvestment: The SHARE Initiative
The Transformation Center collects and reviews SHARE spending plans and develops
SHARE Initiative guidance for CCOs.
Behavioral Health Integration
Behavioral Health Integration Resource Library
The Patient Centered Primary Care Institute hosts an online library of state and national behavioral health integration resources, including virtual site visits and topic-specific interviews with Oregon providers.

Oral health integration
Health Management Associates conducted an environmental scan of both local and national efforts toward oral health integration and conducted extensive interviews to assess current transformation efforts. The white paper below includes recommendations for strategic approaches to further integration, and the oral health toolkit includes resources for supporting oral health integration in Oregon.

CCO incentive metrics
The Transformation Center provides targeted technical assistance for high-priority CCO incentive metrics. Current technical assistance opportunities, webinar recordings and resources for the following metrics are available at
Metrics Technical Assistance:
- Adolescent immunizations
- Diabetes (HbA1c poor control and oral evaluations for adults with diabetes)
- Emergency department utilization among members experiencing mental illness
- Kindergarten readiness
- Oral health (preventive visits for ages 1-14 and oral evaluations for adults with diabetes)
- Well-child visits (ages 3-6)
- System-level social-emotional health
Meaningful language access to culturally responsive health care services
Screening, brief intervention and referral to treatment (SBIRT)
- Tobacco cessation/cigarette smoking prevalence
The Transformation Center has previously provided technical assistance on the following topics, and a number of resources remain available:
- Adolescent well-care visits
- Colorectal cancer screening
- Controlling high blood pressure
- Developmental screening and follow-up
- Effective contraceptive use
- Timeliness of postpartum care
Cross-cutting initiatives
Statewide CCO Learning Collaborative
The Transformation Center convenes the
CCO learning collaborative for CCO medical directors and quality improvement coordinators every other month during the Quality and Health Outcomes Committee meeting.
Transformation and Quality Strategy analysis
The Transformation Center collects and analyzes CCO Transformation and Quality Strategies. The center also provides Transformation and Quality Strategy technical assistance.
Innovation Café
The Transformation Center routinely hosts in-person world-cafe' style events such as the
Innovation Café. In the past, these peer-to-peer events have focused on behavioral health integration, early learning and the social determinants of health.
