Metrics technical assistance

The Transformation Center is currently offering technical assistance to CCOs, clinics and other interested health system partners for the following incentive metrics:

• Diabetes (HbA1c poor control and oral evaluations for adults with diabetes)
  
• Kindergarten readiness
• Oral health (preventive visits for ages 1-14 and oral evaluations for adults with diabetes)
• Well-child visits (ages 3-6)
• System-level social-emotional health
  
• Screening, brief intervention and referral to treatment (SBIRT)
  
• Social determinants of health: Social needs screening and referral
  
• Tobacco cessation/cigarette smoking prevalence
  

The Transformation Center has previously provided technical assistance on the following topics, and a number of resources remain available:

• Adolescent well-care visits
• Child and adolescent immunizations
• Colorectal cancer screening
• Controlling high blood pressure
• Developmental screening and follow-up
• Effective contraceptive use
• Emergency department utilization among members experiencing mental illness (disparity measure)
• Meaningful language access to culturally responsive health care services
  
• Timeliness of postpartum care
