The Transformation Center is currently offering technical assistance to CCOs, clinics, and other health system partners for the following incentive metrics. For updates, sign up for the Transformation Center's technical assistance email list.

• Assessments for children in ODHS custody
  
• Child and adolescent immunizations
• Meaningful language access (health equity)
  
• Oral health (preventive visits for ages 1-14 and oral evaluations for adults with diabetes)
