Events

  • Coordinated Care Model Summits - Events in 2013, 2014 and 2015.
  • Complex Care Collaborative - Annual events in 2013-2016, including two learning collaboratives and two forums focused on members who are dual eligible for Medicaid and Medicare.
  • Innovation Cafe - Events in 2015, 2017, 2018 and 2019.
  • Medicaid spending program convenings - Annual events in 2019-2025 to support CCOs implementing health-related services (HRS), Supporting Health for All through Reinvestment (SHARE) and in lieu of services (ILOS).
  • Patient-centered counseling trainings - Trainings in 2019 and 2021.

Programs

Funding

  • Comprehensive Primary Care Plus (CPC+) - Federally funded multi-payer advanced medical home model (2017-2021). 
  • State Innovation Model grant - Federal grant from 2013 to 2017.
  • Transformation Fund - A 2013 strategic investment in coordinated care organizations from the Oregon Legislature.

Reports

  • Children's health complexity data - CCO, county and statewide reports for 2019-2023.
  • CCO Transformation Plans for 2013-2015 and 2015-2017. In 2018, the Transformation Plan was replaced by the Transformation and Quality Strategy.
  • Senate Bill 436 Report (2014) - Effective July 1, 2013, Senate Bill 436 highlighted issues related to children’s health care and established guidelines for coordinated care organizations to focus on children’s health, to the extent possible, in their community health improvement plans. This report assessed CCOs' progress.

Other areas of support

  • Behavioral health - Webinars to support CCOs in meeting requirements for children's behavioral health.
  • Care coordination and intensive care coordination - A 2022 learning collaborative.
  • Value-based Payment (VBP) Compact - The Transformation Center staffed the Oregon VBP Compact Work Group, a collaborative partnership to advance VBP adoption across the state.
