Moving to Fourth Edition of
The ASAM Criteria
To maintain certification as an Oregon substance use disorder or co-occurring disorders treatment provider, providers must provide care aligned with "The ASAM Criteria, Fourth Edition" effective July 1, 2027.
- Providers with renewal reviews that occur before this date will have their current ASAM levels of care extended through June 30, 2027.
- To ensure continued eligibility for Medicaid reimbursement, providers must be licensed and/or certified for each level of care approved on their license/certificate.

ASAM Foundational Trainings - Offered through July 1, 2026. For training support, please email OHA the following information:
- The desired version (3.0 or 4.0)
- Name, credentials, and email of staff who need training.
You will receive account details and a training access code by email.
SUD Provider Resources: Memos and technical assistance sessions
ASAM Level of Care Applications.
Main Level of Care Application: Submitted by all licensed and certified programs. Addresses information that applies to all Levels of Care.
Outpatient Application: Submitted by all certified outpatient programs.
Residential Application: Submitted by all licensed residential SUD programs. One application per licensed facility.
Withdrawal Management Application: Submitted by all licensed withdrawal management programs. One application per licensed facility.
