Care Coordination and Intensive Care Coordination

CCO Requirements

Learning Collaborative

Throughout 2022, the Oregon Health Authority (OHA) Transformation Center is hosting a monthly virtual learning collaborative to support Coordinated Care Organizations (CCOs) and other organizations that provide care coordination services to Oregon Health Plan (OHP) members. The learning collaborative will provide support toward understanding and meeting CCO 2.0 care coordination requirements and facilitate sharing of CC/ICC best practices.

This virtual collaborative will be held on the 3rd Thursday each month in 2022, from noon to 2pm.

To participate: If you work for a CCO, Kepro, or another organization that is involved with planning or delivering care coordination services for OHP members and would like to participate in this learning collaborative, please contact Alissa Robbins (alissa.robbins@dhsoha.state.or.us) or Tom Cogswell (thomas.cogswell@dhsoha.state.or.us).
