Health-Related Social Needs Provider Training

The Transformation Center hosts monthly trainings for individuals and organizations to become health-related social needs (HRSN) providers and prepare HRSN providers to deliver HRSN services. More information for HRSN providers

Audience: Organizations that want to become HRSN service providers for Open Card/fee-for-service.

Enrolling as an HRSN service provider
  • In this module, we show new providers how to fill out the application to become an HRSN service provider with OHP Open Card/fee-for-service and existing providers how to add on HRSN specialties.
