Use This Form to Ask OHP Client Services for Help.

You can use this form to:

• Ask for an OHP handbook, a Certificate of Creditable Coverage, or a new Oregon Health ID card.
• Ask about changing your coordinated care organization (CCO) enrollment. 
• Share an issue, question, concern or complaint with OHP Client Services.
