Courier Information
The Oregon State Public Health Laboratory (OSPHL) provides courier services to healthcare facilities throughout the state for submission of specimens to our laboratory, contracted to DASH Delivery*. This service is funded through a variety of state and federal resources. Facilities are either set up with a scheduled, regular pick-up cadence or can request pick-ups on an as-needed basis.

Transport Manifest
Include a transport manifest with each package that details all specimens inside the transport container. This helps the lab verify we received each specimen you intended to send.
You may use the
OSPHL Specimen Transport Manifest (pdf) or a format of your choice.
