All specimens submitted for communicable disease testing must follow the OSPHL Specimen Submission Policy.

Clients submitting laboratory orders using electronic data transmission should not complete the paper Test Request Forms.
Occasionally, an outbreak or emerging infection leads to the OSPHL publishing additional or special submission instructions.
