PCPCH Program Impact Report
The 2024-2025 PCPCH Program Impact Report provides an overview of the program's reach, activities, and impact over the past two years. It explores characteristics of the primary care practices that are recognized as PCPCHs, ways in which the program has influenced how they care for their patients, effects on health care expenditures and utilization, the most recent updates to the model (such as those focused on improving health equity), program-led technical assistance, upcoming priorities, and more.
PCPCH Program Health Equity Initiative
In 2021 the PCPCH Program launched an initiative to identify barriers to health equity within Oregon's primary care system in order to guide improvements to the PCPCH model and standards. The initiative included a literature review, health disparity data analysis, and interviews with over 25 community-based organizations, practices, and agencies within Oregon. Findings and implications are included in the 2021 PCPCH Program Health Equity Initiative Report. In 2025, the PCPCH program published a supplemental Health Equity Initiative Final Outcomes: Revisions Included in the 2025 PCPCH Criteria document which summarizes what changes and new additions were included in the 2025 version of the PCPCH model as a result of the feedback received from these partners during the 2021 Health Equity Initiative.
PCPCH Program Evaluations
Primary Care Spending in Oregon
Senate Bill 231 (2015) required the Oregon Health Authority and the Department of Consumer and Business Services to report on the percentage of medical spending allocated to primary care carriers, PEBB, OEBB and CCOs.
