CCO Community Health Assessment (CHA) and Community Health Improvement Plan (CHP) Reports

CCO submissions

See below for CCOs' most recent CHA, CHP and CHP progress report submissions. CCOs develop a new CHP every three or five years, depending on the needs of their community partners. CCOs also submit an annual CHP progress report.

At a glance: tracking community health priorities and progress

  • This CHP Metrics Tracker provides an overview of CHP metrics from 2019-2024 CHP data, including progress, health priorities, and yearly trends. It helps in understanding the focus areas and effectiveness of health initiatives across different service areas by CCO region.

Community health priorities

Community health priority matrix

The OHA Transformation Center, in collaboration with the OHA Public Health Division, developed a matrix of community health priorities by agency and region. This tool supports community collaboration with CHA/CHP partners, based on community health improvement plan priority areas. It is not meant to reflect CCO compliance with contractual requirements.

The matrix only includes community health priorities for community health improvement plans that were completed by March 2021 and will be updated at least annually. The matrix does not include priority areas from other CCO, LPHA or hospital plans. If your CCO, LPHA or hospital notes an error in the priorities within the matrix, please contact the Transformation Center

Story map

To further support collaboration and alignment, the Office of Rural Health partnered with OHA’s Transformation Center and Public Health Division to create an interactive story map (updated August 2020) that links to the most recent plans for each Oregon county. Future versions may also include lists of plan priorities by organization that aligns with the matrix. Please contact Sarah Anderson with the Office of Rural Health with story map feedback or questions.
