Dental Directed Payment Incentives

Bonus Payments for Coordinated Care Organization (CCO) Dental Providers

Oregon Health Authority (OHA) is seeking federal approval of dental directed payments for CCOs. CCOs will use the payments to give incentives to dental providers in their network.  

The incentives are meant to:

  • Increase preventive dental care provided to CCO members to pre-pandemic levels.
  • Maintain and expand the CCO dental workforce.
  • Support community efforts to engage local dental providers and help members access dental care.

To Qualify:

Dental providers must:

  • Be contracted with CCOs to serve CCO members.
  • Provide preventive dental services to CCO members during calendar year 2025.

Types of Incentives 

There are three options for CCO-contracted dental providers. CCOs will receive payments to distribute the incentives in 2026. 

  • New Dental Provider Annual Incentive: New dental providers who sign up and provide service in 2025 are eligible to receive an annual bonus. The bonus varies by services provided.
  • Provider Retention and Increased Access Incentive: Existing dental providers who retain service levels from 2024 are eligible for an annual bonus. The bonus can also increase if services provided in 2025 increase from 2024 levels.
  • Bundled Payments for Pediatric and Minimally Invasive Services (once per year, per member): 
    • Dental providers who complete a pediatric preventative bundle will receive an add-on payment of $30 per bundle.
    • Dental providers who complete a minimally invasive bundle will receive an add-on payment of $50 per bundle.

To Become a CCO Dental Provider:

Timeline

2024

OHA submits Directed Payment Preprint to Centers for Medicare & Medicaid Services (CMS) for approval. View Oregon's approved state-directed payments.

2025

During this year dental providers will qualify for incentives if they:

  • Join CCO networks 
  • Provide preventive care to current patients who are CCO members
  • Provide preventive care to new patients who are CCO members

2026

OHA will provide CCOs directed payments based on member utilization of preventive dental care. CCOs will distribute incentives to participating providers.
