Enroll in OVERS
If you are new to OVERS, follow these steps to get an OVERS account so you can use the system.
- Complete the OVERS Enrollment Form
Include two pieces of ID along with the OVERS Enrollment form.
One piece of ID must have the applicant's full name, photo, and address. (See the enrollment form for more information.)

Submit the completed and signed enrollment form and two pieces of ID either by:
After your enrollment form has been processed and your account is created, you will be emailed by a Center for Health Statistics (CHS) employee with information about how to login to OVERS.
Please note, it may take several days to set up your account. Upon receiving your username and password, you can start using OVERS.
Recommended Training and Resources
Medical certifiers can sign up for an individual training session or complete online training resources.

Find out if your facility has a facility administrator. Facility administrators can assist medical certifiers by monitoring record queues, reviewing records, looking up user information, and resetting passwords.
