The Public Employee Benefits Board (PEEB) is hosting flu and COVID vaccination clinic for state employees and their dependants. Getting a flu vaccine is the best way to stop the spread of flu and protect yourself, your family and your coworkers from influenza and related complications. Everyone six months and older needs a flu vaccine every year. Per the CDC, it is safe to receive the flu shot and the COVID booster at the same time.

Make an appointment
Please use this Microsoft Bookings form to schedule an appointment. You must fill this out for each individual in your household or family that will be joining you.
- Children can be accommodated with parental/guardian consent.
- Kids on Medicaid who are VFC-eligible can receive their immunization.
Please request your time at least 48 hours prior to your preferred day. You will receive an automated email with a calendar reminder confirming your appointment. The email you receive will also have a link you can use if you need to cancel or reschedule your appointment.
You may print, fill out and bring the insurance authorization form/s with you to your appointment. Alternatively, the forms will be available for you to fill out when you arrive for your vaccine(s).
Please remember to bring your insurance card to your appointment.
