Become an Oregon Health Authority Approved THW Training Program

Organizations or individuals who want to provide training or continuing education to Traditional Health Workers (THWs) must apply for approval from the Oregon Health Authority (OHA). 

Important Update

Starting August 2025, OHA will review training applications quarterly. Plan ahead to meet these deadlines by submitting applications at least 120 days before the quarterly meeting dates listed below. OHA will be providing preview of the curricula 45-days upon receiving the complete application once it submitted. If there are items missing, this may delay the review.  

Upcoming Review Dates:

  • May 28, 2026 - FULL
  • August 6, 2026 - Submit application by June 7
  • November 5, 2026 - Submit application by September 6

The following are the applications. Please download the correct evaluation and complete the forms. Save as PDF when completed. Important note: there are checklist within each application to help guide you.
  • Initial Training Program Application - use this application if you are requesting to become an OHA-approved THW foundational training program.
  • Renewal Training Program Application -  use this application if you are a currently approved THW training program seeking recertification. The document contains a checklist of required documents. 
  • Waiver - use this form if you are an OHA-approved training program and need to request an extension.
  • Continuing Education Application - use this application when submitting a request to become an OHA-approved continuing education training program.

Evaluation Rubric - When completing the application be sure to use the evaluation rubric below to help you meet each area. Certificate of Completion Template provides samples of Certificate of Completion and Certificate of Participation which are requirements for the training programs. These templates are important to use to follow guidelines and support future registration.

Wait for Review

You will receive a confirmation email. Within the first 120 days, OHA will be providing prereview of the curricula 45-days upon receiving the complete application. During this time, if applicant does not respond in a timely manner (i.e. two weeks), there will be a delay in the review.
Once approved:
  • Your program will be listed on the official THW training page.

Submit Quarterly Reports to document Impact

You must submit quarterly reports using the THW Training Quarterly Report. These reports are important to capturing workforce impact, numbers trained and more.
