What Is a Phase 3 Provider?
All health care providers must begin collecting and reporting REALD data in accordance with
current REALD standards and Oregon Disease Reporting rules starting January 1, 2022. These include:

Phase 1 and
Phase 2 providers who have not yet begun REALD data collection and reporting for COVID-19 encounters;
- Other health care providers defined in House Bill 4212 (1st Special Session) who do not work in a Phase 1 or Phase 2 health care setting.
OHA encourages Phase 3 sites to plan for REALD reporting prior to the January 1 start date.
Any Phase 3 providers who cannot start reporting data on January 1, 2022, must submit a compliance plan to OHA.
