REALD & SOGI - Providers and Partners

The broad requirement that all providers and insurers report REALD & SOGI data to OHA under ORS 413.161 is not currently in effect. However, some organizations may still have to report this data if it's required by the specific programs they participate in. It's a good idea to check with those program contacts to make sure you understand what reporting requirements apply to you.
The REALD and SOGI data standards have been codified in the OARs (Chapter 950, Division 30).

Implement REALD & SOGI Data in Your System

Collect Data 

Asking REALD and SOGI Questions:
When collecting demographic data using the REALD & SOGI survey, all questions must be asked. Patients may select "Don't know" or "Don't want to answer" for any question. Providers may also choose to adapt their own systems and forms to incorporate the REALD and SOGI questions into their existing data collection processes. If doing so:
  • Do not change or remove wording in the questions or in the response choices for each question.
  • Changing the question order is acceptable.

  • OHA REALD & SOGI Templates One Pager - The purpose of these templates is to see an overview of the survey questions and age-based skip logic for a variety of settings. The templates are not intended to be printed and used as paper forms. The REALD & SOGI Demographic Survey is currently being designed and translated and will be shared here when available.
