REALD for Health Care Providers

Requirements for Health Care Encounters

House Bill 3159 (2021) requires most health care providers and insurers to collect and report REALD information once annually (every 365) for all patients. 

Use OHA's REALD collection templates for health care settings.
  • All questions must be completed.
  • Patients may decline to answer any question.
Until translations of the updated forms are available::
  • Please collect data using the posted translations.
  • Clearly identify the patient’s first name, last name, date of birth and medical record number (if applicable) on each completed form for your records.
Providers may also choose to adapt their own systems and forms to incorporate the REALD questions into their existing data collection processes. If doing so:
  • Do not change or remove wording in the questions or in the response choices for each question.
  • Changing the question order is acceptable.​​

If providers cannot comply with data collection and reporting requirements by the timeline outlined in rule 333-018-0011, they may request an extension by submitting a Complia​nce Plan​ to OHA. ​

The plan must provide sufficient detail to demonstrate a good faith effort to come into compliance in a timely manner, including plans for:

  • Workflow design, 
  • Staff training, and 
  • Any phased implementation​.

OHA's Compliance Plan Template​ lists the required and optional information to include in the plan for OHA review.

OHA will review the plan and may ask for additional information or clarification. ​

Once OHA approves the Compliance Plan:

  • ​Any adjustments to an approved Compliance Plan are subject to re-approval by OHA. 
  • OHA will require monthly reports outlining the progress towards implementing REALD data collection and reporting.
