CCO incentive metrics

OHA uses quality health metrics to show how well publicly funded programs are improving care, making quality care accessible, eliminating health disparities, and curbing the rising cost of health care. OHA tracks a set of incentive metrics, in addition to state performance metrics, to monitor progress. For CCOs, outcome and quality measures have been developed by OHA. Funds from a quality pool are awarded to CCOs based on their annual performance on these incentive measures. The Transformation Center may provide TA to support improvement of incentive measures. Current (2019) incentive metrics include:

Adolescent well-care visits
Ambulatory care: emergency department utilization
Assessments within 60 days for children in DHS custody
CAHPS composite: access to care
Childhood immunization status
Cigarette smoking prevalence
Colorectal cancer screening
Controlling high blood pressure
Dental sealants on permanent molars for children
Depression screening and follow-up plan
Developmental screening in the first 36 months of life
Diabetes: HbA1c poor control
Disparity measure: emergency department utilization among members with mental illness
Drug and alcohol screening (EHR-based SBIRT)
Effective contraceptive use among women at risk of unintended pregnancy
Oral evaluation for adults with diabetes
Patient-Centered Primary Care Home enrollment
Timeliness of postpartum care
Weight assessment and counseling in children and adolescents
