Oral health integration

CCOs are required to integrate oral health into the coordinated care model. CCOs, oral health care providers, and community partners across the state are working together to improve oral health for children and adults.

Example strategies:

  • Integrating oral health in primary care settings (for example, well-child visits, prenatal care visits and as a component of managing chronic conditions)
  • Aligning work with the Oregon Oral Health Coalitions’ Oral Health Strategic Plan, the state health improvement plan, local oral public health assessments, community oral health coalitions, Early Learning Hubs and other population health initiatives
  • Implementing preventive oral health services for infants and toddlers through the First Tooth Project
  • Facilitating shared use of electronic health records and meaningful use with patient-centered primary care homes
  • Facilitating referrals for primary care services when appropriate or inversely facilitate referrals for dental services
  • Expanding the number of settings in which dental care is provided for and reimbursed (for example, in schools, community centers, WIC, mobile dental vans and nursing homes)
  • Increasing access to dental sealants by partnering with state and local programs that provide school-based dental sealant services
