Health-Related Social Needs Housing Benefits

Housing benefits are a health-related social needs (HRSN) service. HRSN housing benefits are for Oregon Health Plan (OHP) members with certain health conditions.

  • Not all OHP members will qualify for HRSN housing benefits. To qualify, you must have specific health, living, and financial conditions.
  • HRSN housing benefits aren’t meant for emergencies. It takes time to approve and receive HRSN housing benefits. Times can vary by service and provider. If you’re facing imminent eviction, you should contact your regional Community Action Agency (CAA).

What Are HRSN Housing Benefits?

  • Help paying rent for up to six months. This includes late payments.
  • Help paying for utilities like gas, electric and water. This includes setup and late payments.
  • Up to six months of storage fees.
  • Tenancy support – help understanding your lease, speaking with your landlord and referrals to other services.
  • Home changes for safety – pest control, heavy duty cleaning, and installation of ramps, grab bars, and drawer pulls.
  • Home changes for health during extreme weather – air conditioners, heaters, air filters, mini fridges and portable power supplies.

Who Qualifies for HRSN Housing Benefits?

Each housing benefit has different requirements. They may include things like your income, your age or having a lease. But for all of them you must be an OHP member to qualify for HRSN housing benefits.

How to Ask for HRSN Housing Benefits

HRSN housing benefits are not meant for housing emergencies, such as an imminent eviction notice. Wait times to receive HRSN housing benefits may be different for each coordinated care organization (CCO) and open card (fee-for-service) OHP.

If you’re applying for help paying rent, you need to have this paperwork ready:

  • Your Oregon Health Plan ID card
  • A lease or rental agreement
  • Proof of income for the past two months like pay stubs

Coordinated care organization (CCO) members:

Ask your CCO. You can call your CCO or use your CCO's HRSN housing request form. Make sure to submit the request to your CCO.

Fill out an HRSN Housing Request Form. Email the completed form to ORHRSN@acentra.com. Completing this form does not guarantee eligibility for HRSN housing benefits.
