2025-2029 State Health Improvement Plan

The State Health Improvement Plan improves the health of all people, of all ages, and all places in Oregon.


We draw on community voices, experiences, and ideas to shape state health focus areas and solutions so that all people living in Oregon have the resources and opportunities to thrive. It is important to recognize the government-to-government relationship between the state of Oregon and the Nine Federally Recognized Tribes in honoring Tribal sovereignty and the role Tribes play in improving health in Tribal communities.

SHIP Collaborative

The SHIP Collaborative is a group of community members from across Oregon who will spend the next four years learning about the 2025 - 2029 State Health Improvement Plan (SHIP) and Oregon’s public health system.
This group may help raise awareness of the SHIP in their communities and build connections with others in public health.

Goals of the first year:

  • Learn how Oregon’s public health system works.
  • Learn the roles of the OHA public health division, Tribes in Oregon, local health departments, hospitals and CCOs.
  • May share how SHIP strategies are impacting their communities.
  • Learn about SHIP goals and progress.
  • May share SHIP efforts and updates with their communities.
  • Choose the group’s purpose and scope for the second year.
