Genetic Counseling Program - License Information

Genetic Counselor License

To be considered for a genetic counselor license, an applicant must:

  • Meet the requirements of Oregon Administrative Rule, Chapter 331, Division 30.
  • Submit a completed application form, which must be accompanied by payment of an application fee of $150.
  • Submit two forms of acceptable identification issued by a federal, state or local government agency. Both forms of identification must include the applicant's current legal name and at least one of the forms of identification provided must be photographic. If submitting by mail, front and back of legible (clear) photocopies must be provided. Submit copies on a full-sized piece(s) of paper, do not cut the ID images out.
  • Be at least 18 years old.
  • Arrange for official documentation to be sent directly from the American Board of Genetic Counseling (ABGC) or the American Board of Medical Genetics and Genomics (ABMGG) to the Health Licensing Office showing proof of an actice credential specific to genetic counseling. Proof must be issued or mailed directly to the Health Licensing Office from the ABGC or the ABMGG.
  • Submit required license fee of $50.

Genetic Counselor Temporary License

To be considered for a genetic counselor temporary license, an applicant must:

  • Meet the requirements of Oregon Administrative Rule, Chapter 331, Division 30.
  • Submit a completed application form, which must be accompanied by payment of an application fee of $150.
  • Submit two forms of acceptable identification issued by a federal, state or local government agency. Both forms of identification must include the applicant's current legal name and at least one of the forms of identification provided must be photographic. If submitting by mail, front and back of legible (clear) photocopies must be provided. Submit copies on a full-sized piece(s) of paper, do not cut the ID images out.
  • Be at least 18 years old.
  • Submit official transcripts from an Accreditation Council for Genetic Counseling (ACGC) accredited program. The official transcript must be issued or mailed directly to the Health Licensing Office from the accredited program.
  • Submit verification from the American Board of Genetic Counseling (ABGC) that the applicant is in "active candidate status". Verification must be issued or mailed directly to the Health Licensing Office from the ABGC.
  • Submit required license fee of $50.
